Annual Health and Medical Form This is a link to the BSA Annual Health and Medical Web page. The first download is Part A&B and the second download is parts A B & C. There are also downloads for physical forms for all High Adventure Bases. This document is needed for scouts to go on outings with the Troop. Part A & B are needed for all outings.
